After you complete a book that qualifies for the monthly theme, you will fill out this form to submit to earn points. This form also contains a few reflection questions and a place to record any Lindenwood DEI events, trainings, etc... you have attended that month or since the last time you submitted a form. Points Info: 20 pts = completing a book from a monthly theme and submitting the corresponding monthly points form 15 pts = for attending the DEI Reading Challenge Fall Semester Event or the Spring Semester Event 10 pts = for attending monthly discussion 5 pts = attending other Lindenwood DEI Events, Trainings, Webinars, etc...
